Important notes to fill out the form below:

• For your dedication to be included in Cathedral News on the date requested, please submit your request the calendar month before your desired date. On this form, include the date requested and your dedication message.

• Please make your donation prior to filling out this form, either by check, payable to the Cathedral of St. John or online as a 'one-time donation' under the Giving menu.

• To cover the cost of both altar arrangements, please donate $150.

• To cover the cost of one arrangement, please donate $75.
